Fire investigators are releasing new details into the death of Phoenix Firefighter Brad Harper. Harper was killed at a fire scene two months ago after he became pinned between a fire truck and an ambulance.

According to the report, this was a horrible accident. Three mistakes played a role in what happened.

Firefighters should a spotter when the fire truck backed up, and they didn't. The firefighter driving the fire truck should have noticed Brad Harper on the road. And Harper should have noticed the fire truck moving towards him.

Phoenix firefighter Brad Harper was pinned between this fire truck and ambulance. Trapped in such a narrow space, firefighters were afraid to move either truck and hurt Harper even more. It took rescue crews 13 minutes and 22 seconds to pull Harper to safety.

"I don't know if I heard someone yelling, I don't know what I heard… I heard something to where... I stopped," said Shayne Arthur in a recording. He was the firefighter driving the fire truck that night.

Arthur's crew told to go to another spot. He was backing down a narrow road.

Meanwhile, Brad Harper was standing on that same road outside of his ambulance, putting his gear, when the fire truck hit him -- pinning him between the two trucks.

Arthur: "Nate asked me if I wanted a backer. I said no, I was already moving. Brian was on the radio and I said no I got it."

Phoenix Fire's policy requires firefighters to use a spotter when backing up. A picture taken of a sticker in the fire truck reminds firefighters to use a backer.

The fire truck was moving at 5 miles an hour when Harper was hit.

There is still an internal fire investigation going on. That team will decide if there should be any disciplinary action or changes to policy to prevent something like this from happening again.
